NOS Revers-O-Matic from Petri Products, Pomret Conneticut and looks like some type of mirror extension or replacement for possibly a better view. No cracks in the glass but starting to get black specks mainly on the right hand side plus one mark/spot about in the middle plus some cloudiness in the mirror.
